In Aivie and Mautic there are different field types for a single answer or multiple answers. Checkbox group and radio group.

Checkbox group

A checkbox group is used to allow several selection options. In the form, this field type creates a square selection box.

A radio group is used to allow a single selection option. In the form, this field type creates a round selection box.
